Endovascular Intervention: Current Controversies

Shropshire, England: tfm Publishing, 2004. ISBN 1-903378-31-1. Softcover, $55.00; pp 312; 100 illustrations.

This volume is a product of the multidisciplinary Endovascular Forum sponsored by the Vascular Surgical Society of Great Britain and Ireland and the British Society of Interventional Radiology held in Stratford, England, in 2004. The 64 authors are vascular surgeons, interventional radiologists, and practitioners of related disciplines. The majority of the contributors are based in the United Kingdom, the Netherlands, and Ireland.

The 33 chapters are organized into seven different sections that cover abdominal aortic aneurysm, thoracic aortic disease, carotid disease, lower limb disease, renal and mesenteric disease, imaging, and venous disease. This provides a broad cross section of many different topics of interest to the endovascular specialist.

The chapters take the form of concise papers, most of which cover a topic in fewer than 10 pages and include a modest number of illustrations, graphs, and tables. Some chapters are presented as brief review articles, whereas others take the form of peer-reviewed original articles of varying focus, reflecting the multiple-contributor nature of this symposium.

The chapters on stent-grafts give an excellent overview of the rapid development and progress in this area. The findings of the European trials and databases provide some interesting insights into the outcomes of early stent-graft technology, despite having been written prior to the publication of the EVAR (Endovascular Aneurysm Repair) and DREAM (Dutch Randomized Endovascular Aneurysm Management) trial results. Chapters such as those covering the RETA (Registry for Endovascular Treatment of Aneurysms) database and endovascular specialist training appear to be specific to the United Kingdom and, as a result, may be of limited interest to North American readers.

The articles are not meant to be detailed, in-depth treatments, and chapters such as the one on vascular malformations are too general for the subspecialist in the field. There are a variety of interesting chapters that address vena caval filters, varicose veins, closure devices, venous thrombectomy, thrombolysis, and even implantable venous valves. However, current controversies relating to subintimal angioplasty are not covered, which is somewhat surprising.

The text is published as a large softcover–format volume; my copy, after being reviewed by a few members of my department, has a rather worn and creased appearance, a testament to the appeal of many of the chapters contained within. Considering that this is a symposium book rather than a standard reference text, the paperback format is preferable to the more expensive hardcover option.

This book will no doubt be of interest to individuals interested in the rapidly evolving procedures of endovascular intervention. Interventional radiologists, vascular surgeons, and fellows in these disciplines will all find material to keep them up to date and abreast of recent developments at a modest price.
